You are required to have the following items ready before completing this application

• Photograph of your vehicle from 3/4 front (example below)
• Photograph of your vehicle from 3/4 rear (example below)
• Photograph of your vehicle Chassis Number
• Safety Cage Certificate (if safety cage fitted and previously certified)
• Safety Cage Manufacturer’s Number (if safety cage fitted and not previously certified)
• Three photos of your cage, one from driver’s side door, one from passenger side door with drivers seat removed, and one showing the structure in the space above the occupant’s head (if safety cage fitted and not previously certified).
